1. You will be asked to provide your name, health card # without the VC and date of birth to confirm your identity and to match our clinic records.

2. Select the desired date and time for your planned visit. You may choose the reason of the visit and add a brief description if needed.

3. You will get an e mail confirmation after a successful booking with the option to confirm, cancel or reschedule in the future.
